Incoming student health requirements

Visit the to submit your health requirements. Choose "香港六合彩资料" and log in with your 香港六合彩资料 information.

Step 1: Click on 鈥淔orms鈥 and complete and upload the following:

  • Meningococcal Vaccination Response Form
  • Medical/Health History/Tuberculosis Form
  • Under 18 Consent Form (if you are younger than 18 years)

Step 2: Send in proof you are immune to measles, mumps and rubella by doing one of the following:

  • Have your medical provider sign a copy of your immunization record and send it to us or you can upload the record to the .
  • Have your medical provider complete a copy of the 鈥淐ertificate of Immunization,鈥 which you can print from the Forms page of the health portal, and send it to us or you can upload the record to the .
  • Send us copies of blood tests (鈥渢iters鈥) which prove you are immune to measles, mumps, and rubella

Note: this will require the help of your medical provider or school nurse, so start early. Make sure your name and date of birth is on every page that is sent to us.

Students should check to see if they have completed all the requirements 5-7 business days after uploading their information. Sign in to the and go to the 鈥淚mmunizations鈥 tab. Any item listed under 鈥淢issing Immunization鈥 is still outstanding and should be uploaded. 

VACCINE EXEMPTIONS REQUEST

Without an approved exemption, failure to meet the vaccine requirements may result in consequences ranging from holds that block you from registering for courses to disenrollment and removal from campus.

Students who are not compliant for the health requirements will receive notices through the patient portal indicating that data is missing or 鈥渘o data鈥 is present. These messages may be received even after submitting immunization records, if the record is incomplete or has not been fully processed by the medical staff.

Steps to take 

  • Things you should do before you arrive
    • Bring  a thermometer.
    • Make sure you know about your health history.
    • Carry a list of any medication you take and any allergies you have.
    • Bring your health insurance card or a copy of it.
    • Program the telephone numbers for the Decker Student Health Services Center (DSHSC) (607-777-2221) and Harpur鈥檚 Ferry ambulance service (607-777-3333) into your phone.
    • While not mandatory, we recommend that you visit with your home healthcare provider before coming to campus. You are encouraged to send any important health information to the DSHSC for inclusion in your student medical file.
    • If you take prescription medication on an ongoing basis, be sure to bring enough with you, or have your doctor call your prescription into a local pharmacy. 
    • If you require allergy shots, the DSHSC can give you a list of local providers, as we do not offer this service on campus.
  • Things you need to do before the sixth week of classes

    Undergraduate students only: If you don鈥檛 need the University鈥檚 Student Accident and Sickness Plan, you must submit an online waiver through to the Student Accounts Office by the sixth week of classes. Instructions on how to submit a waiver can be found at the Student Accounts website. Information about health insurance plan benefits is available. If you don鈥檛 submit the waiver on time, you鈥檒l be responsible for paying for the insurance. If you waive the University insurance plan, we鈥檒l assume you don鈥檛 need coverage in subsequent semesters unless you inform us that you do.
